          Yes, I will commit myself to the following level of participation
          in the 40-Day Purpose-Driven Life Campaign between
          October 9 and November 21, 2004.

Please check one of the following levels of participation:

          Level 1-- Daily prayer and general support of Campaign.

  
          Level 2-- Level 1 plus daily reading of Rick Warren's book.

          Level 3-- Levels 1 & 2 plus attending worship every Sunday.

           Level 4-- Levels 1,2, & 3 plus sharing in an existing Sunday
           School class or group, or a new small group, each week.
           Please check one of the following:

                     Sunday School Class or existing group at church;

                     New small group at church;

                     New small group in a home.

           Level 5-- Levels 1,2,3,& 4 plus helping with leadership and
           organizing of special events. Please check any of the
           following you wish to help with:

                     Prayer Times (Help organize prayer times for the
                     congregation to pray together and individually.)
                     Ministry Fair (Help to plan this one-time event to
                     celebrate the existing ministries of our church.)
                     Missions Fair (Help to plan this one-time event to
                     lift up the ways our church reaches out into the world.)

                     Celebration Sunday (Help plan the final day of the
                     Campaign on November 21, 2004.)
